The August issue of the monthly magazine published by AIChE contains an informative article on safety accidents that have been investigated by the U.S Chemical and Safety and Hazard Investigation Board. The author, John F. Murphy, served on the CBI investigation board following a long career at Dow.
Many of the root causes of these safety incidences have been discussed at MTI meetings, in the TAC forum, and discussed in MTI publications. This article clearly demonstrates the value of safety related technical information that MTI develops and makes available to its members and the wider community.
This article is a good way to introduce the link of the performance of materials of construction to safety problems. In particular, the effect of corrosion on process equipment is highlighted.
